Ask anyone in any medical office who comes first and you are likely to get the same answer: the patient. At our practice, we not only say it but display it with our actions. A lot of work goes on behind the scenes in order to provide our patients the best care possible.
We believe understanding how our office works will help you, our patient, feel even more satisfied with your care and visit.
Our practice is open Monday through Friday from 8:00am – 5:00pm, except for standard national holidays.
Scheduling an Appointment
When calling to schedule a new patient appointment, you will need the following:
Patient name, date of birth, patient phone number, insurance provider and group plan ID’s, reason for their visit and physician referral (HMO plans require a primary care physician referral).
You can download the new patient here [new patient paperwork]. If possible, please complete this information prior to your appointment. If you are unable to complete the paperwork prior to your appointment, then please arrive 15 minutes prior to your appointment time. Each information packet also contains a map to our office. Please bring it with you when coming to your appointment as this will be necessary to find the exact location of our building. We will make every effort to accommodate late arrivals, but it may be necessary to reschedule those arriving late in order to keep other patients appointments on time.
Cancelling an Appointment
If you find that you need to cancel or reschedule this appointment, or any subsequent follow-up appointment, we ask that you contact our office at least 24 hours in advance. Patients scheduled for a procedure should provide at least 48 hours notice prior to canceling or rescheduling an appointment. This will allow those who are waiting for an appointment the opportunity to be scheduled in your place.
We realize the importance of your calls and will strive to answer them in a timely manner. When calling to leave a message, please state your full name, date of birth, phone number and reason for your call. We occasionally get calls without this information or the call was not legible due to poor reception and therefore we are unable to return your call. Please call us back if you do not hear from us. Please allow us 24-48 hours to return non-emergent calls.
